ACGME Case Log (ADS) Login & Submission Guide
Sign in to the official ACGME Case Log in ADS yourself, in your usual browser. The Capnolog extension then prefills the form one case at a time from what you already logged on your phone. You check every field and press the ACGME Save button. It never handles your password.

If you searched for an AAMC case log, an ADS case log, or an ACGME case log login, you found the right place. The official ACGME Case Log System is where those cases belong. ACGME describes the system as a web application within its Accreditation Data System (ADS). It is the system of record. The Capnolog browser extension types it for you — one case at a time, and you press Submit yourself. It never submits for you.

Ask the app for the link
In Capnolog, open the Log tab and tap the ACGME card. That opens ACGME sync. Tap “Email me the extension link.” Open that email on the computer you file from, and add the extension there in Chrome or Edge. A Safari version is on the way.
It needs a Capnolog account, and extensions install on a computer — so send the link to the machine you actually file on.

Connect it once
Back on ACGME sync, tap “Generate code.” Type the code Capnolog shows into the Capnolog extension in your browser. You only do this once.
The extension never handles your ACGME password and never logs in for you.

Open the ACGME Case Log in ADS
Log in to the official ACGME Case Log site yourself, in the browser you normally use. The extension only runs on that site.

Your cases are ready
Everything you’ve logged in Capnolog is already there. It pulls up the first case and fills in the form for you — one at a time, never a bulk run.
Haven’t connected the extension yet? Open the Log tab in Capnolog, tap the ACGME card, then tap “Generate code” and type the code into the extension. Connecting is the only way your cases reach the extension. You can still export your case log as a CSV for your own records or to enter into ACGME by hand.

Review every field, then Save
Check each prefilled field against the case. If a form option doesn't match cleanly, the extension stops and asks you — it never silently guesses. When it looks right, you click the ACGME Save button, and the extension advances to the next case.
You legally attest to every case you submit — so every Save stays in your hands.
