The best case log app for residents in 2027

Capnolog is the pick for anesthesiology residents and fellows. Chris Cutler, an anesthesiology resident, makes Capnolog and wrote this guide, so read the ranking with that in mind.

Each app was read from its own App Store listing, site, or policy on October 30, 2026. The comparison, with its method, prices and limits, is published as a citable preprint: doi:10.5281/zenodo.23817571. A slide deck of the same comparison is on Gamma: view the slides.

What a case log app has to do

  • Count against the ACGME minimums. Each case should land in its defined categories as you log, nested categories included.
  • Get the case into the ACGME Case Log. The ACGME Case Log System is the system of record, so a log that stops short of the form is a second copy to type.
  • Keep patient identifiers out. No names, no medical record numbers, and no free text sitting on a server longer than the case needs.
  • Take less than a minute between cases. A log that takes longer than the hallway walk gets done at midnight, from memory.

The ranking

  1. 1. Capnolog

Best overall for anesthesiology residents and fellows

    • A case is logged on the phone in seconds, and case detail is parsed on the phone.
    • Every case counts against the ACGME minimums as you log, nested categories included.
    • In anesthesiology, the ACGME page inside the iPhone app prefills the official Case Log form in your own account. You review the case and press Submit.
    • No patient identifiers are collected, and the backup is encrypted in your own iCloud.
    • Logging, export, and progress against the minimums are free. The first 50 filed cases are free, and filing after that is part of Premium.

Where the app falls short: Anesthesiology is live. Anesthesiology fellowships and neurosurgery are in beta, and the other surgical specialties are in build. The app runs on phones only, and Android is by invite.

  2. 2. Anesto

    Best for dictating anesthesia cases

    • An iPhone case log from Pierogi Labs, built by a practicing cardiac anesthesiologist.
    • Each case shows the anesthesiology minimums the case counts toward, nested categories included.
    • The listing says the app fills in the ACGME form from the case, sends the form, and checks that the case landed.
    • Case text is read by Anthropic's Claude and voice by Groq, once you agree. The privacy policy keeps case descriptions in its cloud for up to 12 months.

Where the app falls short: iPhone only, and anesthesia only.

  3. 3. RVU Edge

Best for surgical residents outside neurosurgery today

    • A case and wRVU tracker for iPhone and Android that looks up procedure codes with their RVU values.
    • The listing tracks ACGME case categories for 14 surgical specialties.
    • Data is stored on the device and synced to your own iCloud.
    • Cases export to PDF, Excel, and CSV. The site describes a Chrome extension that fills board certification portals.

Where the app falls short: The ACGME Case Log form is not one of the portals the site names, and the app has no anesthesia module.

  4. 4. UNIRA

Best for operative notes beside the log

    • An iPhone app for surgeons that logs cases by procedure code and writes operative notes. The app also helps prep the next day's cases.
    • The app imports ACS Surgeon Specific Registry exports and spreadsheets.
    • Exports include per-CPT breakdowns for boards and the ACGME.

Where the app falls short: iPhone only, with a limited web dashboard. The listing does not describe counting against the ACGME minimums.

  5. 5. Follie

Best anesthesia reference with a simple log

    • An anesthesia reference app from Attuned Labs for iPhone and Android, with a browser version.
    • The case log tracks procedure, technique, rotation, and role, and exports to CSV.
    • The app also generates anesthetic plans and keeps cards for surgeons and the anesthesia team.

Where the app falls short: The listing does not mention the ACGME.

Not ranked: the systems of record

These are where a case ends up or what a program licenses, so a resident does not choose them.

  • ACGME Case Log System

The ACGME's own Case Log System, a web application inside ADS, with the ACGME Case Logs app for iPhone. What you enter there is submitted, and access is enabled by your program.

  • ABOS KSB+

The American Board of Orthopaedic Surgery's app and web portal. Residents in onboarded programs log their ACGME cases through the portal, and residents sitting ABOS Part I from July 2026 must take part.

  • New Innovations and MedHub

Residency management systems a program licenses. Both have a phone app with a procedure logger, and both sign in with the program's account.

Questions residents ask

Does the ACGME endorse a case log app?
No. The ACGME's non-endorsement policy says the ACGME does not endorse, approve, or recommend any data collection product. Capnolog is independent and is not affiliated with the ACGME.
Which case log app gets a case into the ACGME Case Log from the phone?
Capnolog does in anesthesiology: the app prefills the form in your own account, and you press Submit. Anesto's listing says the app fills in and sends the form.
Which case log apps keep case detail on the phone?
Capnolog parses case detail on the phone. RVU Edge's listing says data is stored on the device and in your own iCloud. Anesto keeps case descriptions in its cloud for up to 12 months.
Is there a good case log app for Android?
RVU Edge and Follie list Android builds. Capnolog's Android build is by invite. The ACGME Case Log System runs in any browser.
