Capnolog is a case-logging app for residents and fellows, made by a resident who keeps the same ACGME log. It is one person's work, not a venture-funded team, and everything on this site is written to be checkable against what the app actually does.

Why it exists

Case logging competes with the next case. It gets done in a hallway, at the end of a shift, or in a backlog on a Sunday — and by then the detail that made the case worth logging is gone.

The ACGME Case Log System remains the system of record, and that is the right place for the record to live. It is not the place to find out, in the middle of your third year, which categories you are still short of.

So Capnolog does two things. It takes the case in the seconds after it ends, on the phone already in your pocket. Then it counts what you have against the published minimums for your specialty, so the gap is something you can see in November rather than in June of your final year.

What it is built to

Five commitments, and where each one is shown working.

  • There is nowhere to put a patient identifier.

    The structured case record has no field for a patient's name, record number, or date of birth, because a case log does not need one. Planning notes you type yourself are the disclosed exception, and they are encrypted on your device.

  • Logging is free and stays free.

    Recording cases and exporting your own record cost nothing, on every plan, for as long as Capnolog exists. Premium is a layer on top of the log, never a gate in front of it, and a program that buys the console is buying the console — not its residents' logs.

  • The record leaves with you.

    Your log exports to a spreadsheet and to a readable summary, on your own schedule, without asking anyone. Training ends; the record of it should not be stranded in an app you stopped using.

  • You press Submit.

    The browser extension fills the official ACGME form from cases you already logged, and stops at the button. Nothing in Capnolog submits a case log, because that record is yours to file and yours to check.

  • The website is checked against the app.

    Every load-bearing claim on capnolog.com is registered against the code that makes it true, and the build fails when a page describes something the app does not do. That is why some sentences here are hedged, and why an unshipped feature is labelled instead of quietly described in the present tense.

What Capnolog is not

These are settled decisions rather than a backlog. None of them becomes true by shipping something later.

It does not submit anything for you.
The extension fills the ACGME form and stops. You review what it put there and press Submit yourself.
It does not connect to a hospital system.
There is no chart access, no interface, and no credentials held anywhere. Everything in your log is something you entered or pasted in yourself.
Nobody downstream has agreed to accept it.
Capnolog turns your own count into a document. No program, hospital, board, or credentialing body has agreed to accept that document as a verified record, and only they can decide to.
It does not count unique patients.
Where a requirement counts unique patients, Capnolog counts the encounters you logged. The two numbers diverge whenever you see someone twice, and ours is the larger one.
It does not show you other people's numbers.
Nothing you log is shown to another user. There is no pool of case counts to browse, no leaderboard, and no comparison against your co-residents.
It does not rank programs or predict a match.
A case log cannot model a match outcome and this one does not try. There are no odds, no scores, and no program rankings anywhere on this site.

How it is paid for

Logging and export are free. Premium requires an account and is free during the private beta; at launch it opens with a free trial, and pricing is to be announced.

There is no advertising in Capnolog and personal information is not sold — not to a recruiter, not to a device manufacturer, not to anyone. A logging app funded by selling what residents log would be a different product with the same screens.

What pays for it instead is narrow and stated up front: an optional Premium layer for residents who want the planning, template, and filing surfaces, and a paid console for programs that want their own view. The log underneath both is free.

Where it is today

The specialty modules live today cover anesthesia and neurosurgery, and the rest are listed on the home page, each with the state it is actually in.

The iPhone and Android apps come from one codebase and are both in a private beta: there is no App Store or Google Play listing yet, and access is arranged through the form on the download page. A Mac companion for the desk is in development and is not available to anyone.

Who is behind it

Capnolog is built and run by one person, in training, alongside the log it is meant to make faster. There is no support tier and no queue: the address below reaches the person who writes the code.

Capnolog is a trade name of Chockstone Labs LLC, an Ohio limited liability company, which operates the service.

If something in the app is wrong, or a sentence on this site does not match what you see on your phone, that is worth an email. Both have happened, and both got fixed.
